Probleme der narzißtischen Balance in therapeutischen Institutionen

Die Mitarbeiter in therapeutischen Institutionen sehen sich vielfältigen und widersprüchlichen Anforderungen ausgesetzt: Sie sollen sich mit ihrer jeweiligen Patientengruppe identifizieren, gleichzeitig aber auch ihre eigene Behandler-Identität aufrecht erhalten. Sie sind Teil einer speziellen Berufsgruppe, aber auch einer Gesamtinstitution. Sie sollen offen für Veränderungen innerhalb der Institution sein, gleichzeitig aber auch Stabilität und Sicherheit garantieren. Sie sollen gesellschaftliche Delegationen erfüllen, die speziell im Maßregelvollzug unvereinbar erscheinende Pole umfassen: Sicherheit durch Absonderung und Heilung durch Integration.

Gefährdung und Förderung dieser prekären narzißtischen Balance werden in dieser Arbeit auf drei Ebenen untersucht: In der Beziehung von Therapeuten- und Patientengruppe, in der Beziehung unterschiedlicher Mitarbeitergruppen untereinander und in der Dynamik der Gesamtinstitution und ihrer Beziehung zur Außenwelt.

Abstract

Those who work in therapeutic institutions feel themselves to be exposed to manifold and contradictory expectations: They are supposed to identify themselves with the group of patients with whom they are working at any given time, but simultaneously they are expected to maintain their own identity as the person responsible for the treatment. They are members of a special profession, but are also part of a complete institution. They are expected to be open towards changes within the institution, but at the same time they also have to guarantee stability and security. They are supposed to fulfill duties delegated to them by society, which encompass polarities which are not compatible, particularly with regard to the implementation of disciplinary measures: Security by way of segregation and healing by way of integration. Endangering and promoting the precarious narcissistic balance of this situation is examined on three levels in this work: In the relationship between therapist groups and patient groups, in the inter-relationship between groups of different types of working personnel and in the overall dynamics of the whole institution and its relationship to the ‘outside world’.

Résumé

L’ensemble du personnel travaillant dans des institutions thérapeutiques se voit confronté à des exigences diverses et contradictoires: Ils doivent s’identifier à leur groupe de malades respectifs tout en gardant cependant leur propre identité de thérapeute. Ils font non seulement partie d’un groupe professionnel spécial mais aussi de l’institution dans sa globalité. Ils doivent être ouverts à tout changement dans le cadre de l’institution, tout en garantissant également stabilité et sécurité. Ils doivent assumer - par délégation - des tâches sociales qui, en particulier dans le “Maßregelvollzug” allemand semblent être incompatibles les unes avec les autres: Assurer la sécurité par l’isolement et la guérison par l’intégration.

La mise en péril et l’encouragement de cette balance narcissique précaire seront, dans le cadre de cet article, analysés sur trois niveaux, à savoir au niveau de la relation entre les thérapeutes et le groupe de malades, au niveau des relations entre les différents groupes professionnels et au niveau de la dynamique de l’institution dans son ensemble et de sa relation avec le monde extérieur.
