Psychodynamische Aspekte der klinischen Behandlung von Gewalt- und Sexualverbrechern

Der Autor stellt ausführlich die Vorgehensweisen der institutionellen Psychotherapie im holländischen Maßregelvollzug am Beispiel des Pieter-Baan-Centrums vor. Er bezieht sich ausdrücklich und ausführlich auf die psychoanalytischen Theorien über die frühe Kindheits-entwicklung als Referenz für die Gestaltung des therapeutischen Klimas in der Klinik. Dieses therapeutische Klima ist Grundvoraussetzung für die bei dem Patienten zu weckende Hoffnung auf eine Verbesserung. Die Qualität dieses Klimas dient auch als Grundlage für die Möglichkeit, einen psychotherapeutischen Zugang zu dem Patienten zu ermöglichen. Hierbei besteht durchaus die Gefahr maligner Regressionen für den Fall, daß die psycho-therapeutische Arbeit zu früh, d.h. zu einem Zeitpunkt, in dem der Patient noch nicht in ausreichend intensive, soziotherapeutische Kontakte einbezogen ist, einsetzt. Zum anderen verweist er auf die Gefahr lediglich formaler Anpassung für den Fall einer rein medikamentösen Behandlung oder ausschließlich stützender Einzelgespräche ohne psychotherapeutische Aufarbeitung der Reinszenierungen bei den hier angesprochenen Patienten mit erheblicher Gewalt- oder Sexualdelinquenz.

Abstract

The author gives a detailed introduction into the method of procedure in institutional psychotherapy in the implementation of disciplinary measures in Holland as in the example of the Pieter-Baan-Centre. He particularly refers to psychoanalytic theories of early development in childhood as the reference point for creating the therapeutic climate in the clinic. This therapeutic climate is a basis pre-requisite for hopes of improvement, which have to be awakened in patients.

The quality of this climate also serves as a basis for the possibility of being able to find psychotherapeutic access to the patient. However, herein exists the danger of malignant regressions in the event of the psychotherapeutic work being put into operation too early, that means at a time at which the patient is not yet adequately intensively involved in contacts of a social-therapeutic nature. On the other hand the author points to the pitfall of mere formal conformity in the event of a purely medical treatment or completely supportive individual conversations without a psychotherapeutic working out of the repetions of the primal scenes with patients, whom we have referred to in this article as being extremely violent and sexually delinquent.

Résumé

L’auteur présente, en détail, les différents procédés au niveau de la psychothérapie institutionnelle dans le cadre de la psychiatrie légale hollandaise à travers l’exemple du centre “Pieter-Baan”. Il se réfère, de façon expresse et circonstanciée, aux théories psychanalytiques sur l’évolution de la petite enfance comme référence pour l’organisation d’un climat thérapeutique dans la clinique. Ce climat thérapeutique est la condition fondamentale pour éveiller, chez le malade, l’espoir d’une amélioration. La qualité de ce climat sert également de base, base qui permet de favoriser un accès thérapeutique auprès du malade. Dans ce cadre, le danger de régressions malignes existe réellement au cas où l’on commence le travail psychothérapique trop tôt, c’est-à-dire à un moment où le malade n’est pas encore intégré dans des contacts sociothérapiques suffisamment intensifs. D’autre part, il attire l’attention du lecteur sur le danger d’une adaptation purement formelle dans le cas d’un traitement exclusivement médicamenteux. Il met également en garde contre les simples entretiens d’assistance sans aucun travail de réflexion psychothérapique sur les remises en scène chez ces malades ayant une délinquance sexuelle ou agressive importante.
