“The difficult road back” - Problems of reintegration of forensic psychiatric patients in or near their original surroundings

Von 1989 bis 1993 wurde in einem Modellprojekt des Westf. Zentrums für Forensische Psychiatrie Lippstadt versucht, für ehemalige forensisch-psychiatrische Patienten eine Nachbetreuung durch stationäre und ambulante Dienste zu organisieren. Das Projekt zielte darauf ab, die regionalen psychiatrischen und psychosozialen Dienste durch koordinative Hilfen, Fortbildungsangebote und die örtliche Präsenz qualifizierter Mitarbeiter in die Lage zu versetzen, auch für diese Klientel Betreuungsaufgaben zu übernehmen.

Es wird berichtet, welche Hindernisse sich aus der Sicht regionaler Dienste einer solchen Betreuungsübernahme entgegenstellen und welche Strategien zur Überwindung dieser Hindernisse entwickelt wurden.

Eine Auswertung der Nachsorgebedarfe von 35 Patienten, die vor und während der Laufzeit dieses Projektes in die Städte Herne und Bochum entlassen wurden, zeigt, daß ca. jeder 3. Patient nicht die bei der Entlassung indizierte Nachsorge erhielt. Mittels einer Arbeitszeitanalyse wurde der Zeitaufwand ermittelt, der zur Organisation der Nachsorge und für konkrete patientenbezogene Betreuungsmaßnahmen erforderlich ist.

Abstract

Between the years 1989 and ’93 the Westphalian Centre for Forensic Psychiatry in Lippstadt initiated a pilot-project through which former forensic psychiatric patients could receive after-care either with in-patient or out-patient services. Through co-ordinated help, through further educational facilities and through the presence locally of qualified personnel the project was aiming to enable the regional psychiatric and psycho-social services to take over the task of looking after these types of patients.

Pertaining to the obstacles which face an undertaking of this kind, a report is given from the point of view of the regional services, indicating which measures were developed to overcome these impediments.

An evaluation of the after-care needs of 35 patients, who were dismissed prior to or during this project in the towns of Herne and Bochum, shows that approximately every third patient did not abtain the after-care assigned to him at his dismissal. In an analysis of working time, it was assertained how much time was needed for the organization of an after-care service and for actual patient-related measures.

Résumé

Entre 1989 et 1993, le Centre de Psychiatrie Légale d’Eickelborn a entrepris de mettre sur pied, dans le cadre d’un projet modèle, un suivi social se composant d’offres d’aides ambulatoires ou hospitalières faites à d’anciens malades de psychiatrie légale. Ce projet visait à ce que les services psychosociaux et les services psychiatriques régionaux soient capables - grâce à des aides coordinatives, à des offres de formation continue et à la présence de personnel qualifié sur place - de s’occuper de tâches touchant au suivi social de cette clientèle.

L’article dresse, d’après les services régionaux, le rapport des obstacles auxquels a été confrontée cette prise en charge du suivi social et relate les stratégies qui ont été développées pour faire face à ces obstacles.

L’étude des besoins en suivi social de 35 malades qui ont été -avant et pendant la durée de ce projet - admis dans des établissements de réhabilitation situés dans les villes de Herne et de Bochum, montre qu’environ un malade sur trois n’a pas été admis dans l’établissement de réhabilitation indiqué. Au moyen d’une analyse du temps de travail, on a pu établir le temps de travail qui est nécessaire à l’organisation générale du suivi social et à celui nécessaire aux mesures concrètes de suivi social ayant attrait aux malades.
