Zusammenfassung

Vorgestellt werden ausgewählte Ergebnisse der externen Evaluation des Projektes „Ambulante Nachsorge“ des Westfälischen Zentrums für Forensische Psychiatrie Lippstadt. Mit diesem Projekt, in dessen Rahmen 35 entlassene und 28 beurlaubte Patienten betreut wurden, sollten bedarfsgerechte Nachsorgestrukturen und -maßnahmen für ehemals gem. § 63 StGB untergebrachte Patienten geschaffen werden. Im Vordergrund stand die Überleitung von Patienten in nachsorgende Einrichtungen und Dienste sowie die (fallbezogene) Zusammenarbeit der Klinik u.a. mit komplementären Einrichtungen, Bewährungshilfe und Führungsaufsicht. Im Rahmen des Projektes gelang die Neuakquisition zahlreicher Betreuungsangebote. Die Ergebnisse der externen Evaluation verweisen auf die hohe Bedeutung eines klaren, verbindlichen Procedere im Rahmen der Überleitung von Patienten und der intensiven Begleitung durch die Nachsorgemitarbeiter der Klinik. Auf Grundlage der Ergebnisse zur fallbezogenen Kooperation im Rahmen „Runder Tische“ wird die Frage der Zuständigkeit bzw. Fallverantwortlichkeit für (bedingt) entlassene Patienten erörtert.

Summary

This article presents selected results of the external evaluation of the project “Outpatient after-care” of the Westphalian Centre for Forensic Psychiatry, Lippstadt. The central goal of this project, during which 35 released patients and 28 patients released on trial were cared for, was to establish structures and strategies of after-care appropriate to the needs of patients formerly in indefinite detention. The main focus of the project was on the integration of patients into outpatient psychiatric and psycho-social institutions as well as on the (case by case) cooperation of the clinic with these institutions, probation services, other legal authorities etc. The project succeeded in establishing numerous new modes of cooperation with psychiatric and psycho-social institutions in the field of after-care. The results of the external evaluation indicate the importance of a transparent, reliable process in the transition from inpatient treatment to outpatient aftercare, and of continuing personal support for released patients by social workers of the clinic. The question of responsibility for (conditionally) released patients is discussed on the basis of results concerning the case by case cooperation within “Round Tables”.

Résumé

Cet article présente un échantillon des résultats issus de l’évaluation externe dans le cadre du projet de suivi médical ambulatoire du centre de psychiatrie légale de Lippstadt en Westphalie. L’étude a porté sur 35 patients libérés d’hospitalisation et 28 patients en congé d’essai. Le projet visait à mettre en place des structures et des mesures pour un suivi adapté de patients qui dans un premier temps ont été placés selon l’article 63 du Code Pénal allemand. Il s’agissait principalement d’orienter les patients vers des structures post-hospitalières et le cas échéant de la coopération de l’hôpital avec entre autres des structures complémentaires spécialisées, le bureau de probation et d’autres services juridiques. Ce projet a permis de regrouper un grand nombre de structures ou de services permettant la prise en charge. Les résultats recueillis dans le cadre de l’évaluation externe montrent l’importance de la mise en place d’une procédure claire et fiable dans le transfert des patients, et aussi l’importance d’un suivi rapproché par les équipes hospitalières en charge du suivi ambulatoire. Sur le fondement des résultats de la collaboration au cas par cas dans le cadre de «tables rondes», la responsabilité générale et individuelle par rapport aux patients libérés (à l’essai) est discutée.
