NeuhausChronic graft rejection is primarily responsible for the unsatisfactory long-term results after organ transplantation. The pathophysiological mechanisms involved remain poorely understood and may include alloantigen dependent factors as HLA-incompatibility and acute rejection episodes as well as a variety of alloantigen-independent factors including ischemia and reperfusion injury, viral infections and donor/recipients criteria. However, the potential interaction of those risk factors remains unknown. We present an overview of the current knowledge on chronic graft dysfunction based on recent clinical and experimental reports. Characteristics of transplant vasculopathy and mechanisms leading to this process are discussed in addition to chronic graft rejection in other organ systems.
