121 patients who took part in the European multicenter trial comparing tacrolimus and a cyclosporine-based quadrupel induction regimen as primary immunosuppression between 1990 and 1992 were followed up 36 to 50 months after liver transplantation. Both immunosuppressive protocols demonstrated good organ- and patient-survival with low rejection rates (5-year patient survival tacrolimus 82%, CsA 85%, 5-year organ survival tacrolimus 77%, CsA 75%). OKT3-resistant and chronic rejection episodes occurred less frequent under tacrolimus (1.6% vs. 11.7%). The incidence of infections, neuro-, and nephrotoxicity as well as de novo-malignancies was comparable in both groups.
