Combined Liver Islet Transplantation: Indication and Problems

Combined liver islet transplantation is a procedure for replacement of the function of liver and endocrine pancreas. It is indicated for the treatment of malignant tumors following upper abdominal exenteration or in liver transplantation in type I diabetics. This procedure seems not to induce complications exceeding those after isolated liver or islet transplantation. Sixteen operations have been done so far, 6 in diabetics and 10 in patients with carcinoma. The longest period of functioning islets observed in a diabetic was 49 months. The longest survival of a patient treated for tumor by upper abdominal exenteration was more than 42 months with a still functioning islet transplantat.
