Cardiac Beta-Adrenergic Receptor Function and its Responsiveness in Children with Dilated Cardiomyopathy

Background: Children with idiopathic dilated cardiomyopathy(IDC) always had poor cardiac systolic function, which was thought to be related to the reduction of cardiac ß-adrenergic receptor (ß-AR) in number and in responsiveness of cardiac ß-AR to ß-AR stimulation. The purpose of this study was to evaluate cardiac ß-AR function and its responsiveness in 10 children with IDC and therefore to select a suitable regimen for these patients.

Methods: Blood pressure and heart rate were monitored, left ventricular systolic function and end systolic volume index(ESVI) were measured in children with IDC by using echocardiography and dobutamine stress test was applied to these patients during our study.

Results: ESVI was higher in children with IDC than that in controls, and ratio of systolic blood pressure and ESVI, ejection fraction, fractional shortening, and changes of left ventricular posterior wall thickness were significantly decreased in IDC children compared with those of controls. The cardiac ß-AR response in children with IDC to ß-AR agonist dobutamine was decreased, demonstrating an impaired ß-AR function in children with IDC.

Conclusion: Dobutamine stress test is useful in assessing cardiac ß-AR function and its responsiveness clinically and in selecting a suitable therapeutic option for children with IDC.
