Abstract:
We intend to analyse the immediate and recent results in mitral
valve repair using the quadrangular ressection of the posterior
leaflet without ring annuloplasty. Between the period of January
1984 and december 1996, 118 patients underwent mitral valve
repair with this technique. Age ranged from 30 to 86 years
(average of 59,1 +/- 11,8 years), and 62,7% were male. Associated
procedures were performed in 22,0% of the patients, the most
frequent was coronary artery bypass grafting (15 patients
12,7%). In 20 patients (16,9%) we used associated techniques of
valve repair, most frequent was chordal shortening (6 patients).
Immediate mortality was 0,9%. The linearized rates for
thromboembolism, endocarditis, reoperation and death in the late
post- operative period were respectively 0,4%, 0,4%, 1,7% and
2,2% patients/ year. The actuarial survival was 83,8 +/- 8,6% in
12 years, freedom from reoperation was 91,8 +/- 4,3%,
endocarditis 99,2 +/- 0,8% and thromboembolism 99,2 +/- 0,8% in
12 years. In the late post- operative period, 93,8% of the
patients were in NYHA functional class I, with a complete follow-
up of 89,7%. We conclude that patients with mitral insufficiency
due to prolapse that underwent mitral valve repair with this
technique, presented a satisfactory evolution in 12 years.
